To create a ticket from your home screen, click on the Envelope that says Submit a Ticket from the home page or hit the Submit a Ticket button at the top righthand side of the page. That will take you to the ticket form. See image below:
Once on the create a ticket page choose 'Create A Ticket' from the drop-down menu:
You will fill in the fields as follows :
